| Important Information! - Monday, July 06, 2009ALL TEAMS: Please fill out registration form and e-mail upon completion to choyle@hotmail.com
Checks are due one week prior to tournament start date.
You must have a copy of a birth certificates for all children on your roster in case of protest.
All teams must carry their own insurance and provide proof of it prior to the tournament. Exception; if the manager can show proof of Insurance/release that every child on the roster has insurance and will not claim any injury against KASS that will be acceptable also. If a child does not have medical insurance that child will not be eligibale for game play. If you carry this kind of waiver it must state that KASS is not responsible for anything including Co-pays, Etc. You must provide KASS's medical release and your own. This must be faxed a long with KASS's and your own. You may be asked for proof so please keep it on you at the tournament.
Official rosters are due before your first game.
